For Patients

Contacting your insurance company

Patients with private insurance (not Medicare or Medicaid) must contact their insurance company prior to surgery for authorization of services. Contact them immediately after your procedure is scheduled.

After speaking with your insurance provider, please call our office. If your insurance company gives you an authorization number, pass that information along to us; we will need that number for your records and to ensure proper payment for services. In some cases, our office will need to speak with your insurance company directly. Please advise us of this.

Failure to notify your insurance company could result in a denial of service, in which case, you would be held accountable for payment in full.

If Medicare or Medicaid is your primary insurance company, you will not need to contact them.
