Fremont Health is more than a community hospital. It’s a healthcare system that includes a full-service medical center, primary care and specialty clinics, as well as specialized physicians and services.

On September 1, many of Fremont Health physician clinic locations became outpatient departments of Fremont Health Medical Center. This transition is referred to as Provider Based Status. 

There will be no change in how patients receive their care or which physician/provider they see. Scheduling appointments and phone numbers remain the same. There are some changes that affect how the clinics bill patients. Most of the clinics will also have a new name effective September 1. 

The following outpatient clinics will transition to Provider Based Status along with a new name and new signage:

  • Fremont Health Behavioral Medicine (previously Senior Solutions/Psychiatric Services)
  • Fremont Health Care for Women (previously Healthcare for Women)
  • Fremont Health Family Care (previously Fremont Family Care)
  • Fremont Health Infectious Disease & Travel Medicine (previously Infectious Disease & Travel Medicine)
  • Fremont Health Internal Medicine (previously Fremont Internal Medicine)
  • Fremont Health Orthopaedics & Sports Medicine (previously Fremont Orthopaedics & Sports Medicine)
  • Fremont Health Surgical Group (previously The Surgical Group)
