For Patients

As a courtesy to our patients, we file most insurance for you. It is important, however, that you contact your insurance company if you have questions about provider participation or pre-approval of services.

On the day of your clinic visit, bring your insurance card and co-payment. If you do not have insurance coverage, payment is expected for services when they are rendered. You will be asked to complete an update form for address, employer, emergency contact, and insurance.

Payment may be made in cash, personal check, MasterCard, Visa, Discover, or a debit card. Patients experiencing financial difficulty may meet with a patient account specialist to discuss payment arrangements.

Workers’ Compensation – if you believe your injury is work-related, please inform us when you check in so appropriate authorization can be obtained from your employer.

Emergency medical needs will be handled by the provider on-call. Please call (402) 727-1091 after hours, for emergencies only.

Routine calls for appointments or medication refills should be done during normal clinic hours. We encourage you to contact your pharmacy for medication refills. Requests that come in after hours will be handled on the next business day.

Our clinics protect your health and personal information. For this reason, we ask that you provide us with a list of people that we may discuss your medical or personal information with. Please do not be offended if we refuse to release information to any individual that you have not authorized. Our entire Notice of Privacy Practices is posted in the waiting area and you may request a copy at any time.
